本人对参加此次赛做以下保证：1、 我保证：遵守比赛组委会所有规则和服从一切裁决。2、 我保证对自己的参赛器材、装备及个人所有物品负有全部责任。3、我对在连续而激烈的比赛中可能发生的危险已有充分的认识，我保证本人的身体健康状况能够承受本次比赛，并具备近期健康检查证明书。4、我保证：本人在比赛中因受伤、遭遇意外事故或发生疾病时，对受到的急救处理方法及结果不提出异议。5、我本人、我的家属、亲戚和保护人已充分理解本次比赛的内容和意义，并认可我本人参赛。在比赛之前我已作了人身保险。因此，我保证：我自己、亲属及遗嘱执行人、财产继承人，对本人在比赛中或参加由本次比赛委会组织的与比赛有关的活动时，不论什么原因所致的伤、病、亡或出现的后遗症等，不对比赛组委会提出任何起诉。所造成的损失由保险公司和本人承担，比赛组委会不再提供因此而造成的损失费。6、我保证：允许比赛组织者自由使用和制作与比赛有关的我的名字、照片等。并承认与本次比赛有关的新闻、广播、电视等商业宣传报道。7、我保证：由于自然灾害等客观原因造成比赛取消或变更比赛时间、内容所引起的有关费用问题，服从比赛组委会的决定；由于我个人的情况而不能参加比赛的，不要求退还参赛时所交纳的各种费用。8、我确认：对参赛所进行的上述保证已充分理解，并承认本保证书解释权属比赛组委会。本人自愿签字。保证人签字：
